Over the last 73 years Conway Medical Center has been successful in its mission, in part, due to its ability to build and maintain long lasting relationships with local corporate entities. During its first 13 years Conway Medical Center Foundation has met with success in its mission of support and growth because of relationships that have been established with corporate and community members. The HEALTHREACH Corporate Partners Program was developed by Conway Medical Center Foundation to further the mission of improving health for the underserved individuals of our community with the assistance of significant corporate partners. This innovative program has been designed to help benefit the lives of many people in our community while affording the opportunity to enhance the public profiles of the partners involved in the process. Partnerships demonstrate to the communities they serve a strong commitment to accountability and innovative ideas, coupled with the entrepreneurial spirit at its best. One of the key elements of a successful corporate partnership is that both entities share some of the same corporate values. Commitments can be made more easily when these values are identified and embraced at the outset. Corporate Partner Levels and Recognition Level I - $ 25,000 Annual Contribution ·Corporate logo prominently included as a part of all promotional efforts about HEALTHREACH offerings to medically under served individuals throughout the year. ·Corporate logo prominently displayed as a Corporate Partner at each site where prevention offerings for medically under served individuals will be held. ·Company specific .30-second co-promotions produced and aired with WPDE TV-15 and WWMB TV-21 during April-August time frame focusing on [Corporate Name’s] significant support of health improvement efforts for medically under served individuals. (Co-promotion frequency = 60X: TV-15 = 40X, TV-21 = 20X. Gross Rating Points = 152, age 24-54, approximate value = $ 6,250.) ·Included in more than 300 video-billboard mentions with WPDE TV-15 and WWMB TV-21 throughout the year promoting HEALTHREACH medically under served offerings. ·Included in annual feature piece in local print media focusing on [Corporate Name’s] significant support of health improvement efforts for medically under served individuals. ·Corporate Partner will be asked to appoint designee who will receive monthly reports on program offerings. Corporate Partner representatives will be invited to an annual luncheon meeting where program offerings from the previous year and plans for the upcoming year will be reviewed and discussed with Conway Medical Center staff members. ·Receive HEALTHREACH Corporate Partner plaques, which will be appropriately designed for display at [Corporate Name’s] local business location(s). ·Special Recognition as a HEALTHREACH Corporate Partner and inclusion in live television coverage at the Annual Kingston Golf Classic. ·Recognition as a Lifetime Fellow by The 1928 Society at Conway Medical Center. .Level II - $ 12,500 Annual Contribution ·Corporate logo included as a part of all promotional efforts about HEALTHREACH offerings to medically under served individuals throughout the year. ·Corporate logo displayed as a Corporate Partner at each site where offerings for medically under served individuals are held. ·Company specific .10-second audio-video billboards aired with WPDE TV-15 and WWMB TV-21 during April-August time frame highlighting [Corporate Name’s] significant support of health improvement efforts for medically under served individuals. Gross Rating Points = 76, age 25-54, approximate value = $ 2,649.) ·Included in more than 300 video-billboard mentions with WPDE TV-15 and WWMB TV-21 throughout the year promoting HEALTHREACH medically under served offerings. ·Included in annual feature piece in local print media focusing on [Corporate Name’s] significant support of health improvement efforts for medically under served individuals. ·Corporate Partner will be asked to appoint designee who will receive monthly reports on program offerings. Note: Corporate Partners will be encouraged to make a three year commitment with the understanding that the Partners’ level of participation can change during that three year period.
